How to File a Grievance
Employees working under the BellSouth Telecommunications and BellSouth Utilities contracts have 60 days from the day they discover the contract violation to have the grievance met on at the informal level and file for the 2nd step, the formal level.
You can file a grievance by printing and filling out the following form and faxing them to the Local at 772 468-6606.

STATEMENT OF OCCURRENCE |
The Statement of Occurrence documents the contract violation. This is where you provide the details of your case. Your signature on the form gives CWA the authority to view your records on your behalf. Once the Statement of Occurrence is received into the Union office, it is logged in, assigned a case number and the request for the records we need to win your case is sent to the appropriate 2nd level manager.
